Living Longer, Living Stronger with Chronic Conditions

Living Longer, Living Stronger with Chronic Conditions is Areawide Aging Agency's health promotion and disease prevention program. The Chronic Disease Self-Management program a six-week workshop developed at Stanford University to teach persons living with chronic conditions and caregivers caring for those with chronic conditions strategies to improve their own health - one step at a time. By attending the workshops participants have learned tools to help them manage pain, fatigue, set realistic goals, and make daily tasks manageable. The workshops are FREE to persons over 60 and their caregivers.
Funding for the program is provided in part by a grant from the Adminstration on Aging to the Aging Services Division of Oklahoma Department of Health and Human Services.
